Abstract

Background: Drug-resistant tuberculosis (TB-RO) is still a health threat in the world, plus there are cases that do not respond to the administration of rifampicin. Diagnosis of Rifampicin Resistant TB (TB RR) requires a multidisciplinary approach, markers such as Interferon gamma (IFN-ɣ) are useful for the diagnosis, follow-up, therapy, and prognosis of TB RR. Hypoalbuminemia accompanies TB patients, as seen from weight loss and albumin in the serum of TB patients. TB infection stimulates cytokines that impact on decreasing the regulation and synthesis of albumin, as well the prognosis and success of TB therapy, especially RR TB. This study aims to determine the role of albumin supplementation on increase of albumin serum level, increase of CD8+ lymphocyte cell count, and increase of IFN-ɣ level in RR TB patients.

Methods: This True Experimental Pre and Post-Test Control Group Design research was conducted at Saiful Anwar Hospital. The subjects were 30 people, consisting of 10 people in the Healthy Group (Control Group), 10 TB RR patients were given Albumin (egg white extract) for 30 days (RR TB albumin), and 10 TB patients were not given Albumin (RR TB Non albumin). Samples were taken from peripheral blood, before and after the administration of ATD and Albumin, then analysed with Flowcytometry technique. Data result analysed with SPSS and PLS.

Results: There were differences in albumin and IFN-ɣ levels before supplementation, between the control group and RR TB patients. There was no difference in the number of CD8+ lymphocytes (p=0.402) and IFN-ɣ levels (p=0.390) in RR TB patients who were given albumin or not, but there were differences in albumin levels (p=0.003) and weight gain (p=0.014).

Conclusion: Albumin administration has the effect of increasing albumin levels and weight gain, but has no effect on CD8+ lymphocyte cell counts and IFN-ɣ levels in RR TB patients.
